Health Exam Requirements
Per Illinois state law and District 47 board policy 7:100, children entering preschool, kindergarten, 6th and 9th grades must have a completed physical examination and proof of vaccination on file with the school health office before the first day of school. In addition, all children NEW to Illinois public schools (regardless of grade level) must provide proof of a physical, dental and vision exam.
Completed health forms may be scanned and emailed to the school (see medical records submission) or mailed to or dropped off at the school office on or after August 2, 2022. Forms may not be sent to the district office (CORE center).

Physical exams
Who: Incoming preschool, kindergarten and 6th grade students
What: Physical exam performed by a licensed doctor, nurse practitioner or physician’s assistant
When: Before the first day of school. Physical exams must have been completed within one year prior to the first day of school. Children without proof of a current physical exam will not be allowed to start school per District 47 board policy 7:100.
Dental exams
Who: Incoming preschool, kindergarten, 2nd and 6th grade students
What: Dental exam performed by a dentist
When: By May 15th of current school year. The dental exam must have been completed within 18 months of the May 15th deadline.
Vision exams
Who: Incoming preschool and kindergarten students
What: Vision exam performed by child’s eye doctor (Note: Primary care doctors/nurse practitioners cannot perform the vision exam.)
When: By October 15th of current school year. The eye exam must have been completed within one year of the October 15th deadline.
Sports physical exams
Who: 6th, 7th and 8th grade students who choose to participate in middle school athletics
What: Sports physical performed by licensed doctor, nurse practitioner or physician’s assistant
When: Prior to start of sport; sports physicals are valid for 395 days
