Registration

To register your child for the 2026-27 school year (Grades K-8), please follow the steps below. If you have any questions, please contact Student Services at 815-788-5000.

Fill out and submit the 2026-27 New Student Registration Questionnaire. Within 48 business hours after submitting the form, you will receive an email from District 47 with the online registration link.
After you submit your child's online registration, you will be contacted by student services to set up an appointment to finalize your registration and bring in the required documents outlined below.
- A legal guardian must provide several registration documents and a photo ID. For information on the documents required, please refer to Registration Information and Requirements below.

Other Information

Student Fees may be paid by cash/check or online via ParentVUE and are not due at the time of registration. If you have questions, please get in touch with Student Services at 815.788.5000.
Student physical/immunization forms are not due at the time of registration. Per Illinois state law and District 47 board policy 7:100, children entering preschool, kindergarten, 6th and 9th grades must have a completed physical examination and proof of vaccination on file with the school health office before the first day of school. Medical forms must be dropped off at the school and will not be accepted at the district office (CORE center). To submit your child's medical forms electronically, visit the medical records submission for the appropriate school email address. Visit the Health Services page for a list of the state immunization and examination requirements by grade level.

Students attending school in District 47 must live within district boundaries. Upon registration, parents/guardians must show proof of residency by providing one of the following documents in their name(s):

Purchase contract* with property address and signatures of both buyer (parent/guardian) and seller

Property tax bill in parent/guardian name (look up your tax bill here)
Copy of closing documents* with property address and signatures of both buyer (parent/guardian) and seller
Purchase contract* with property address and signatures of both buyer (parent/guardian) and seller
- *Closings within 30 days: parent/guardian must provide a copy of closing documents or purchase contract within 5 days after closing
Current rental agreement (lease) with property address, signatures of both landlord and tenant (parent/guardian), and expiration date of the lease term*
Notarized affidavit (see below)

Please Note: Parent/Guardians are required to update their residency documents per the guidelines below:

A new/updated lease is required upon the expiration date.
- Leases that are month-by-month or year-by-year: District 47 will add 1 year only to the expiration date.
A NEW affidavit for each NEW school year
When the family moves to a new residence.

Affidavits

A residency affidavit is a legal document that can be used to verify residency. An affidavit must be completed by the homeowner or lease holder and must be notarized. (A notary is a public official who serves as a witness of the signing of important documents.) An affidavit can be notarized at various places in the community or at the District 47 CORE Center at no charge. To schedule an appointment to have your affidavit notarized, please call (815) 788-5000. If the affidavit will be notarized at the CORE Center, the homeowner must be present and bring an ID and proof of residency. Affidavits must be updated annually.

The School District reserves the right to evaluate the evidence presented, and merely presenting the items listed in this
Procedure does not guarantee admission. WARNING: If a student is determined to be a nonresident of the District for whom
tuition must be charged, the persons enrolling the student are liable for non-resident tuition from the date the student began
attending a District school as a non-resident. A person who knowingly enrolls or attempts to enroll in this School District on a
tuition-free basis a student known by that person to be a nonresident of the District is guilty of a Class C misdemeanor, except
in very limited situations as defined in State law (105 ILCS 5/10-20.12b(e). A person who knowingly or willfully presents to the
School District any false information regarding the residency of a student for the purpose of enabling that student to attend
any school in that District without the payment of a nonresident tuition charge is guilty of a Class C misdemeanor (105 ILCS
5/1020.12b (f).

If you do not have one of the above, contact the District office to request an affidavit. *Affidavits are valid for current school year only

Note: Property owners within D47 boundaries who do not live at the property are not considered legal residents. Their children do not qualify to attend school on a tuition-free basis.

Original Certified Birth Certificate

Original birth certificate with an impressed seal from the county or state in which your child was born. *Hospital certificates will not be accepted. **Registering Parent name must be on birth certificate

Kindergarten Eligibility

A child turning (5) years old on or before September 1st of any school year has historically been a requirement for kindergarten.

Students transferring FROM AN ILLINOIS SCHOOL

Must provide one of the following by the 1st date of attendance

Illinois Student Transfer Form (ISBE 33-78.) Provided by your child’s prior school.
Illinois Student Health Physical Form, including Immunizations.

*NOTE: PRIOR to the start of the school, Kindergarten must be provided to the school on or before the first day of school.

Students transferring FROM OUT OF STATE

Must provide the following by the 1st date of attendance

Student Health Physical Form and Immunization that meets requirements of the State of Illinois dated within one year of date of registration.

Other Health Forms:

Illinois Dental Exam Form—Kindergarten, 2nd grade, and 6th grade students: completed by their dentist no later than May 15th of the current school year.
Vision Exam Form— Kindergarten students and any student new to Illinois: completed by an optometrist and submitted no later than October 15th of current school year.

We are moving to Crystal Lake. What do I need to register my children for school?

Your child’s original birth certificate with raised seal
Proof of residency within district boundaries. Acceptable proof of residency includes a current signed and dated lease, or a signed contract/closing documents from a home purchase.
Student Health Physical Form—All new students must have a physical form that meets the requirements of the State of Illinois Health Physical Form.

Middle school sports physicals must be done yearly.

Will I have online access to my student’s information?

If you are the parent who registers using the online registration process, your ParentVUE access will be set up at that time. If you are the non-enrolling parent, you will need to contact your school's office to obtain an activation code for ParentVUE. Access to progress reports, attendance, online yearly registration, and fee payment are a few of the convenient features of ParentVUE.

Will my child have bus transportation to school?

If you live within 1 1⁄2 miles from the school you attend, transportation will not be provided unless your address is in the hazardous areas determined by Illinois Department of Transportation or if you have a special education student and their IEP notes transportation is required. Should your child be eligible for bus service, transportation information will be provided shortly after he/she is registered.
