Change of Address

Currently enrolled families who move within District 47 boundaries must complete a change of address form (English) (Spanish) and provide proof of residency for the new address. Documents can be dropped off or mailed to the district office, located at the CORE Center, 300 Commerce Drive, Crystal Lake, Illinois 60014. Please note the following about change of address procedures:

  • If the change of address involves a change in custodial care, legal documentation must be provided.
  • Address changes will not be made without a completed change of address form and proof of residency.
  • Address change may affect school placement. Children can stay at their current school for the remainder of the school year in which the address change occurred or they can attend the new school on the third school day after notifying the district of the move. This allows the district time to contact the school office to arrange enrollment and complete scheduling, teacher assignment, and transportation (if applicable).
