Change of Address Procedure
Families who move within D47 school boundaries and whose child(ren) are currently enrolled and attending a District 47 school must complete a change of address form and provide proof of residency for the new address. The form and residency documentation should be brought to the school office or to the CORE Center, located at 300 Commerce Drive in Crystal Lake.

- If the change of address involves a change in the family unit, legal documentation of the change will be requested.
- Address changes will not be made without the change of address form and proof of residency.
- Address change may affect school placement. Parents may choose to stay at the previous address so their child(ren) can finish out the year at the current school.
- Students may attend the new school on the third day after notifying the District of the move. This allows the District time to contact the school office to arrange enrollment as well as to complete scheduling, teacher assignment, and transportation (if applicable).
