Affidavits

A residency affidavit is a legal document that can be used to verify residency. An affidavit must be completed by the homeowner or lease holder and must be notarized. (A notary is a public official who serves as a witness of the signing of important documents.) An affidavit can be notarized at various places in the community or at the District 47 CORE Center at no charge. To schedule an appointment to have your affidavit notarized, please call (815) 788-5000. If the affidavit will be notarized at the CORE Center, the homeowner must be present and bring an ID and proof of residency. Affidavits must be updated annually.
