1. Proof of Residency:

If a new family is renting a home or apartment in the Crystal Lake School District, they must bring in their signed and dated lease. This lease must be signed by both the renter and the landlord and have the rental address. If both signatures are not on the lease we will not register the student(s).
If a new family is purchasing a residence in Crystal Lake School District, they must bring their signed contract with buyer and seller signatures and address of residence. If the family has settled on the residence, they must bring in signed closing papers i.e., settlement statement or title. Again, contract must include names and address of residence.

We will not register student (s) without one of these documents.
  2. Also, needed is the original or certified copy of the children’s)
   birth certificate(s).
   Certified copy will have an impressed seal. This is the birth
   certificate from the county or state in which your child
   was born. Due to State of Illinois law,
   HOSPITAL CERTIFICATES will not be accepted.
 

3. The Illinois Student Transfer form (ISBE 33-78) is
    required if your child is transferring from another
    Illinois school. This form can be obtained from the
    previous school of attendance (in Illinois)
 

4. All students moving to our district from out of state,
    regardless of grade level, are required by the State of
    Illinois to have a physical, and must be on
    an Illinois State form. You will also need an
    Eye Exam on the Illinois form as well.
 
5. School assignments are made at the time of registration
    according to your address and our boundaries.
 

6. School records will be requested when your child(ren)
    is/are registered. If your child is receiving any special
    services, please contact our office.
 

7. Busing is provided for students whose residence
    is a mile and a half from their school; this is a state law,
    or if a roadway has been deemed hazardous by the State.
 

8. There is a book rental fee charged each year
    per student. Preschool and kindergarten fees are $40.00.
    Grades 1-5 fees are $60.00 and grades 6-8 fees are $75.00.
    Sixth graders are also required to purchase gym
    shorts for $8.00 and a gym shirt for $9.00. These
    fees are prorated according to month of
    enrollment and are also refundable if transferring out
    of district before the end of the school year.
